Community Health

Program Structure

The Community Health Program consists of Registered Nurses who provide a wide range of nursing services in a variety of settings to Worcester County residents of all ages. Community Health Nurses assess, support, educate, and encourage individuals to be independent in meeting their care needs within a self-care framework. Care also focuses on disease prevention, treatment referrals, and education on personal and family emergency preparedness.
