A combination of free education, support and prevention services taught by staff from Atlantic General Hospital and Worcester County Health Department including a Certified Diabetes Educator/Registered Nurse, Registered Dietitian, and an American College of Sports Medicine Certified Exercise Specialist. 


Diabetes Education Program
 

Free, six-session Diabetes Self-Management Education program provided in Snow Hill and Pocomoke. Team taught by staff from Atlantic General Hospital and Worcester County Health Department including a Certified Diabetes Educator/Registered Nurse, Registered Dietitian, and an American College of Sports Medicine Certified Exercise Specialist. 

Currently scheduled classes:

  • 10-11:30 a.m., Thursdays, July 7-Aug. 11, at the Pocomoke Health Center, 400A Walnut St.
  • 4-5:30 p.m., Wednesdays,Aug. 17-Sept. 21 at the Worcester County Health Department, 6040 Public Landing Road in Snow Hill.
Diabetes Support Group
Free, meets 3rd Wednesday of every month, 1-2 pm, Snow Hill Library and 3rd Thursday of every month, 1-2 pm, Pocomoke Library.  



Diabetes Prevention Program (Lifestyle Balance) 

Free, 16-week, nutrition and physical activity behavior change program designed to help break the family cycle of diabetes. Program meets 3 days per week and is team taught by an exercise specialist and registered dietitian and can be provided at community and faith-based sites in Worcester County. Free incentives, education, and lifestyle balance coach.

Current class: 5:30-6:30 p.m., Mondays and Wednesdays at the Lower Shore YMCA in Pocomoke City (Jan. 10 - May 2).

These services are provided by the Prevention Program.
